1. I have been diagnosed with Obstructive Sleep Apnea (OSA) by completing a sleep study test approved by a registered sleep physician; OR

3. I acknowledge that the Air Liquide Healthcare team is available to offer guidance on modifying my CPAP settings and ensuring the proper use of the device to achieve optimal therapy outcomes.

Portable oxygen concentrators are intended for the administration of supplemental oxygen according to a doctor's prescription or direction. The device is NOT intended for use in life-supporting or life-sustaining situations. In proceeding with the purchase, you confirm that you:

• have consulted your doctor in relation to the use and suitability of this product;
• hold a current and valid prescription from an Australian registered medical practitioner for supplemental oxygen where required; and
• will only use the device in accordance with the instructions contained in the user manual and under the ongoing care and supervision of your doctor.
