By participating in this promotion, you agree to be bound by these Terms and Conditions.

• Offer Period: This offer commences on 18 Nov 2025 at 12.01am and ends on 17 Nov 2026 at 11.59pm, unless extended or terminated earlier by Air Liquide Healthcare.

• The Offer: Any customers can purchase a subscription whereby the purchaser will receive: 

• two (2) Sibionics continuous glucose monitor devices every 28 days; and 

• a Credentialled Diabetes Educator consultation (either in-person at a participating clinic or via telehealth) upon remaining subscribed for designated time periods.

Diabetes Education consultations are offered during the first, third, sixth, ninth, and twelfth 28 day cycles of an ongoing subscription and every three months thereafter.  

A Credentialled Diabetes Educator is a specialized health professional who empowers people with diabetes to self-manage their condition. They provide evidence-based education, clinical care, and support on medication, lifestyle, and technology (including assistance to support safe and effective use of CGM) to improve quality of life. CDEs are registered nurses, pharmacists, dietitians, or other healthcare professionals who have completed rigorous, postgraduate-level training in diabetes education. They must undergo a credentialing program with the ADEA to ensure they maintain the highest standards of care. 

• Subscription Duration: There is no minimum subscription period and the subscription can be cancelled at any time by contacting Air Liquide Healthcare on 1300 36 02 02 or emailing alhdiabetes@airliquide.com. If a subscription is cancelled after dispatch of the next scheduled shipment, then the cancellation will take effect after that shipment.

• Cost: $178 per 28 day cycle, charged via direct debit. 

• Eligibility: This offer is valid for Australian residents only. 

• Offer Conditions and Limitations:

• This offer is available for purchase online and through Air Liquide Healthcare Clinics. The offer cannot be redeemed through pharmacies, wholesalers, or other retail channels.

• This offer cannot be used in conjunction with any other promotion, discount, or offer from Air Liquide Healthcare.

• Credentialled Diabetes Educator consultations must be scheduled within 14 days of the relevant subscription cycle. Appointments cannot be “banked” to be used at a later date. 

• Australian Consumer Law: These terms and conditions are not intended to limit, exclude, or modify any statutory consumer guarantees or rights applicable under the Australian Consumer Law (set out in Schedule 2 of the Competition and Consumer Act 2010 (Cth)).

• Therapeutic Goods Advertising (TGA) Code Compliance: The Sibionics GS3 CGM is a medical device regulated by the Therapeutic Goods Administration (TGA). It is intended for the continuous monitoring of interstitial fluid glucose levels in patients 3 years and older with diabetes mellitus type 1 or 2. You should consult with your healthcare professional to determine if this product is right for you. ALWAYS READ THE LABEL AND FOLLOW THE DIRECTIONS FOR USE.

• Standard Terms of Sale: All products and services are supplied by us in accordance with the Air Liquide ‘Standard Terms of Sale’ available at: https://au.healthcare.airliquide.com/legal. To the extent of any inconsistency between the Air Liquide Standard Terms of Sale and the express terms of this document, the terms of this document will prevail.

• Direct Debit Agreement: By participating in the Offer, you authorise Air Liquide Healthcare Pty Ltd (ABN 41 002 643 045) to arrange for funds to be debited from your nominated account/card for the full Offer cost detailed above. • Device Is Not a Substitute for Medical Advice: The Sibionics CGM is a tool for monitoring glucose levels and is not a substitute for professional medical consultation. The data it provides is for informational purposes only. Air Liquide Healthcare is not a medical provider and does not offer medical advice. All health-related decisions, including diagnosis and treatment plans, must be made in consultation with a qualified healthcare professional.

• Your Responsibilities: By accepting this subscription offer, you agree to operate the Sibionics CGM device according to the provided user manual and all accompanying instructions. You are responsible for seeking guidance from your healthcare professional to understand the device's data. You agree that any decisions regarding your medical treatment or management will not be based exclusively on the information from this device.

• Reporting of Adverse Effects: Should you experience any adverse effects, such as skin irritation or discomfort while using the device, you must immediately discontinue its use and seek advice from a healthcare professional. We also request that you report your experience to Air Liquide Healthcare customer service to assist with our product safety monitoring.

• Modification and Termination: Air Liquide Healthcare may, in its sole discretion, modify these terms and conditions or suspend, postpone, or terminate the promotion at any time. Any changes will be effective when posted on the promotional webpage. Air Liquide Healthcare will notify you of any such change and you will be at liberty to terminate the subscription if you do not agree to the updated terms and conditions.

• Direct Marketing Consent: By registering for this subscription offer, you consent to Air Liquide Healthcare sending you direct marketing communications via email, sms, direct message or other electronic means. These communications may include product support and education, special offers, and updates. You may opt-out of receiving these communications at any time by following the unsubscribe instructions provided in the message.
